What Is Menopause?

Menopause is a natural phase in a woman’s life when her menstrual cycles permanently end, signalling the conclusion of her reproductive years. It is diagnosed when a woman has not had a menstrual period for 12 consecutive months.

Menopause typically occurs between the ages of 45 and 55, though some women may experience it earlier (ages 40 to 45 years) or later (up to 60 years). Menopause can happen earlier due to factors such as:

  • Hormonal imbalances: Hyper/hypothyroidism, hyperprolactinemia and premature ovarian insufficiency
  • Surgical causes: Removal of ovaries
  • Medical treatments: Chemotherapy and radiation therapy that may affect ovarian function

As menopause progresses, the production of key hormones like oestrogen and progesterone decreases, which can result in a variety of physical and emotional symptoms.

Menopause Treatment and Management Options in Singapore

Menopausal Hormone Therapy

Menopausal Hormone Therapy (MHT) is designed to replace the female hormones oestrogen and progesterone, which decrease during menopause. It is considered the most effective treatment for relieving symptoms such as hot flashes, night sweats, and mood swings. MHT also promotes bone health by increasing bone density and reducing the risk of heart-related conditions linked to declining oestrogen levels.

Oestrogen and progestin (the synthetic version of progesterone) can be administered in multiple forms. Here are some of the most common forms for MHT:

  • Oestrogen patch to manage symptoms such as hot flushes, vaginal dryness and bone loss
  • Oestrogen pills such as estradiol and conjugated estrogen
  • Vaginal oestrogen to help with vaginal dryness
  • Low-dose birth control to alleviate symptoms like hot flashes and irregular bleeding
  • Oral progestins such as micronised natural progesterone, medroxyprogesterone acetate, norethindrone, and norgestrel
  • Intrauterine progestin (IUD) to reduce uterine cancer risk

Non-hormonal medications

For those who cannot or prefer not to opt for MHT, non-hormonal medications can be an effective alternative to manage symptoms like hot flashes and sweating. Non-hormonal options include:

  • Antidepressants such as venlafaxine, escitalopram
  • Paroxetine, which can help alleviate hot flashes
  • Gabapentin, a medication commonly used for nerve pain, which is also effective for hot flashes
  • Clonidine, which helps reduce hot flashes and sweating

Lifestyle changes

Making simple lifestyle changes can help ease the symptoms of menopause and improve overall well-being. Here are some practical tips to stay comfortable and support your health during this transition.

  • Maintain a nutritious diet by choosing wholesome foods and keeping hydrated
  • Engage in regular exercise such as walking or yoga to enhance energy and well-being
  • Use a handheld fan or a refreshing water mist to manage hot flashes
  • Wear layered clothing to adapt to temperature shifts easily
  • Incorporate stress-relief techniques like yoga, mindfulness, or meditation

FAQs About Menopause

What is the difference between perimenopause and menopause?

Perimenopause is the transitional phase leading up to menopause, where a woman’s hormone levels, especially estrogen and progesterone, begin to fluctuate. On the other hand, menopause is when a woman’s menstrual cycle permanently ends and the production of key hormones like estrogen and progesterone decreases.

Common physical symptoms of perimenopause and menopause include the following:

  • Hot flushes and night sweats
  • Sleep disturbances and fatigue
  • Aches and pains
  • Headaches
  • Vaginal dryness
  • Low libido
  • Sore breasts
  • Bloating
  • Weight changes
  • Hair changes (i.e. thinning of scalp hair or unwanted hair growth)
  • Skin changes
  • Gum bleeding

Every woman experiences these conditions differently. If you suspect you may be going through perimenopause or menopause, consult our menopause specialist for help in managing the symptoms.

The lowest age for menopause is before 40, which is known as premature ovarian insufficiency. Women with this condition should see an endocrinologist to undergo a comprehensive check of their hormones and further evaluation of their health.

Women who experience menopause between the ages of 40 and 45 years are considered to have early menopause. These women should also undergo a comprehensive endocrine evaluation of their health.

According to research, MHT is a safe and effective treatment for menopausal symptoms in healthy women. It is especially safe for women aged 50 to 60 or those within 10 years of their last menstrual period.

However, some risks may be associated with MHT depending on factors such as age, family and medical history, and the type and dosage of medication. Additionally, some women may experience certain side effects from the treatment such as:

  • Nausea
  • Bloating
  • Fluid retention
  • Vaginal bleeding
  • Sore breasts

Most of these side effects improve within the first three months of treatment. Speak to our menopause specialist about MHT and your concerns today.
