Low testosterone

Low testosterone, also known as hypogonadism, occurs when the body produces insufficient levels of the hormone testosterone. This condition can affect men of all ages, though it is more common as men age.

Low testosterone levels can result from a variety of underlying factors, including:

 

  • Testicular issues: This can include conditions like genetic disorders (e.g., Klinefelter syndrome), undescended testicles in childhood, or injuries and infections affecting the testicles.
  • Cancer treatments: Radiation, chemotherapy, or hormone therapy, particularly for prostate cancer, can impact testosterone production.
  • Medications: The use of drugs like opioids or anabolic steroids may lead to reduced testosterone levels.
  • Chronic medical conditions: Conditions such as obesity, diabetes, liver or kidney disease, and HIV/AIDS can affect hormone levels.
  • Pituitary gland disorders: As the pituitary gland regulates hormone production, disorders affecting it can lead to lower testosterone levels.

 

Symptoms

The symptoms of low testosterone vary depending on the extent of the deficiency and its duration. It is important to note that mild decreases often do not cause symptoms.

Male menopause, or andropause, refers to age-related changes in testosterone levels, typically occurring between 40 and 55 years old, though it can happen earlier or later. Unlike women, men do not experience a clear transitional phase.

 

Causes

Andropause is primarily caused by a natural decline in testosterone levels as men age. By the time men reach the age of 70, their testosterone levels can decrease by up to 50%. Early andropause may be caused by various lifestyle and physiological factors. Some causes include:

 

  • Chronic conditions like diabetes and obesity
  • Medications such as steroids
  • Stress and depression
  • Poor diet and lack of exercise
  • Sleep deprivation

Andropause symptoms can vary from one individual to another, but some common signs include:

Premature ejaculation happens when you reach orgasm and ejaculate sooner than desired during sexual activity. It is a common condition, particularly among men aged 20 to 40.

How Are Andrological Conditions Diagnosed?

Diagnosis of andrological conditions begins with a thorough medical history and physical exam. Further tests, such as blood tests and imaging studies, may be needed to confirm the diagnosis.

For low testosterone, your doctor will discuss symptoms such as fatigue, low libido, and mood changes while evaluating risk factors like obesity, chronic illnesses, and medication use.

 

Key areas of focus for erectile dysfunction diagnosis include sexual history, lifestyle habits, and overall health. You will also be asked about erectile function during masturbation, whether erectile dysfunction developed suddenly or gradually and whether morning erections still occur.

Blood tests are essential for diagnosing erectile dysfunction, andropause and low testosterone. Common tests include measurement of testosterone levels, and checking cholesterol, blood sugar, thyroid hormones and other specialised hormones as required.

In some cases, specialised tests may be necessary to determine the root cause of erectile dysfunction. For example, an ultrasound may be used to evaluate penile blood flow, detecting vascular issues that could impair erectile function.

How Are Andrological Conditions Treated?

When managing andrological conditions, remember to consult an andrologist to develop a personalised treatment plan. Discussing all medications (both prescription and over-the-counter) with your doctor helps prevent potential interactions that could impact your health.

Premature ejaculation

  • Counselling and psychological support help address performance anxiety, stress, depression, and relationship issues through therapy with psychologists, psychiatrists, or sex therapists

  • Medications include antidepressants (e.g. Dapoxetine), erectile dysfunction drugs (e.g. Sildenafil, Tadalafil, Vardenafil, Avanafil), Tramadol, and anaesthetic creams or sprays to reduce sensitivity and delay ejaculation

  • CBT helps manage anxiety and negative thought patterns through cognitive restructuring and mindfulness, improving confidence and ejaculatory control
